Converting the BAP form

I would like to have the current form changed a little and converted over to a word document. But, I don't know how :angry3:

We have wanted to do this for a while.... any help?

I would like it done for the next Jan. meeting. So, no real rush.

My reason... is that in word it would be more email friendly

Is the BAP form available in Word? I found the link to the PDF version, but have not been able to find a Word version.
